1.UnitedHealth Group
UnitedHealthcare sells policies directly to customers in all 50 states, and in roughly half of those jurisdictions, the company also offers individual and family plans through the state-run health insurance exchanges. Minnetonka, Minnesota is home to UnitedHealthcare’s main office.

Centene began in 1984 as a Medicaid plan provider and has since grown to become the largest Medicaid managed care provider in the country.

Humana provides group medical, dental, and vision insurance as well as Medicare Advantage, supplemental, and prescription drug plans for employees. Humana has stopped selling new individual Affordable Care Act policies in January of 2018
